4. Accident Reporting

    ALL ACCIDENTS must be reported IMMEDIATELY to either:

    1. Accident / Incident / Occupational Disease Report Form for employee must be completed by his/her supervisor for any injury/occupational disease involving university faculty/staff. It is to be submitted within 24 hours  c/o: WSIB Administrator, 263 McCaul Street, 2nd Floor or FAX: 416-971-3052.  Form can be found at http://ehs.utoronto.ca/report-an-incident/
    2. Online Accident Form for students / contractors / visitors. The identity of both the accident victim and report preparer must be included. A copy should be sent to OEH&S (Fax:416-971-1361) and to Insurance and Risk Management at FAX: 416-978-5572.  Form can be found at http://ehs.utoronto.ca/report-an-incident/

    Reporting of Critical Injuries

    A critical injury is one of a serious nature which either places life in jeopardy; produces unconsciousness; results in substantial loss of blood; involves the fracture of a leg or an arm; involves the amputations of a leg, arm, hand or foot; consists of major burns to the body or causes the loss of sight in an eye. Besides reporting to the University of Toronto Police at 416-978-2222  and university safety personnel, the Ministry of Labour at 416-235-5330 must be notified.

    In addition to the above legal reporting requirements, the University requires the reporting of all accidents using the appropriate forms for any person, whether or not a personal injury is involved.
